Each consent to share is 2 pages of Easy English.

Signature by you or by another person such as a guardian.

Choose the sections you need for each client.

Where you need multiples of a type, for example health professionals, copy those 2 pages multiple times.

You can personalise from the short list before handing to your client.

You cannot change the Easy English.

Include only the people or professions each client engages with, that you want to request consent. Create your individualised version by

  • including the front page and introduction pages up to the end of page 7, (Your name and where you live)
  • select the 2 pages for each person or professions you need
  • add multiples of the same type of contact: Eg: health professionals
  • add all the pages from this original after the page You may not want to talk about some things, from page 20 to the end.

Bind together, or put 3 staples down the left side of your form. It will now open like a book. Each consent will have the information about the person/profession on the left page and what your client writes for the contact details on the right page.

Note: This form does not have page numbers. It allows you to select only the sections you need, without confusing the reader with missing page numbers.
